MEET DR. CHANDNI CHOUDHARY


Knowledgeable. Patient. Caring. Skilled.

Dr. Chandni did her family medicine residency at Baylor College of Medicine. She is currently board certified in family medicine and is a member of the American Academy of Family Physicians. Upon graduation from Baylor, she took care of the medically underserved population of Harris county. This experience has made her comfortable treating patients with multiple medical and social issues. 

Dr. Chandni’s interests include helping patients lose weight and develop a healthy lifestyle.

She is fluent in English, Hindi, and Urdu. She lives with her husband and two young children. In her spare time, she enjoys spending time with friends and family, watching movies, dining out, and going to the park with her kids.

  • What wellness tests should I have?

    Depending upon your family history and previous medical tests, we may recommend tests that monitor your:


    • Fasting blood sugar
    • Cholesterol
    • Liver function
    • Blood pressure
    • Kidney function

    If you are at a particular risk for a disease—especially if it runs in your family—one of our primary care physicians can help you reduce the chances of getting it.

  • How can I eat a more nutritious diet?

    We admit it: We could all do better when selecting what to eat. It’s far too easy to fall for a “fad diet” only to find yourself back to square one.


    We have extensive training in nutrition and helping you make healthy choices. Just ask one of them to help you create an eating plan. We’ll also provide follow-up appointments to be sure you have the assistance you need to stay the course.

  • What is the best exercise plan for me?

    Please talk to us before embarking on an exercise program. We can help you choose the most beneficial plan for you.   This is especially important if you have health problems or a previous injury.


    We want you to exercise, but we want you to do so safely!


    Remember, you don’t have to become an athlete overnight. Often, even moderate amounts of exercise can go a long way toward fighting diseases such as diabetes and high cholesterol.

  • How often should I schedule an appointment?

    This depends upon your general health, lab tests and your medical history.


    For example, if you have prediabetes, we may suggest that you have your blood sugar tested more often. However, if your lab results look good, then a visit once or twice a year may be all you need.


    Our point is there is no one answer. We’ll work with you to determine how often you should schedule an appointment with us.

  • What is my risk of developing heart disease?

    Half of Americans have 1 of 3 key risk factors associated with heart disease making it the number one killer of Americans. Often, those with heart disease don’t know they have it until they have a heart attack.


    Risk factors include: 

    • High blood pressure
    • High cholesterol
    • Smoking

    Your Lifestyle, family history, and age can also increase your risk for heart disease. This is why it’s so important for us to assess your risk —especially if it runs in your family


  • What are my blood sugar numbers?

    Blood Sugar number, also known as glucose levels, is the amount of sugar found in your body's blood. Your blood carries this sugar to your cells to use as energy. Sometimes your blood sugar can be too low or too high. Your lifestyle and the type of foods you consume can affect your body's blood sugar numbers.


    If you've been diagnosed with diabetes, keeping your blood sugar numbers in the target range advised by a physician is important. 


    Checking your fasting blood sugar levels during a general wellness exam is crucial for your health, as millions of Americans don't realize they have diabetes.


    We help manage and prevent diabetes through dietary guidelines or prescribed medication by checking your numbers.

  • Can you explain these test results?

    We understand— sometimes your paperwork looks like a mixed alphabet. Many abbreviations can make it overwhelming and confusing; terms such as HDL, LDL, and A1C.


    We are here to help you comprehend the test results. We encourage our patients to ask questions and voice concerns if they arise.
