Dr. Chandni Choudhary was born and brought up in India where she obtained her M.B.B.S degree from Lady Harding Medical College. Upon completion of her medical degree she moved to Houston, Texas with her husband.
She did her family medicine residency at Baylor College of Medicine. She is currently board certified in family medicine and is a member of the AAFP. Upon graduation from Baylor, she worked at Harris health taking care of the medically underserved population of Harris county. This experience has made her comfortable with treating medically and socially complex patients with multiple medical and social issues.
Dr. Choudhary’s interests also include helping patients lose weight and develop a healthy lifestyle.
Dr. Choudhary is fluent in English, Hindi, Urdu. She lives with her husband and 2 young children. In her spare time she enjoys spending time with friends and family, watching movies, eating out, going to the park with her kids.


Tiffany grew up in Pearland, TX and received her Bachelor of Science in Microbiology and Biochemistry at Texas State University. During her undergraduate degree, she started the first official Pre-Physician Assistant Organization at the university and served as president for two years. She also did biochemical research working to replicate the p53 tumor suppressor protein in order to aid in cancer research. Her abstract and poster presentation was represented at The Women in Science and Engineering Conference in 2012.
Before Physician Assistant School, Tiffany did an internship at the Johnson Center for Autism Research, studying the serum biomarkers related to autism. She also worked as a Medical Assistant at Central Texas Allergy Asthma and Immunology Center of Austin, TX for three years.
Tiffany attended PA School at Barry University in Miami, FL. During school, she served as secretary of her class, and helped to organize a homeless clinic on South Beach in Miami. Among her experience, she completed three advanced internal medicine clinical rotations, a plastic surgery rotation in which she performed breast cancer reconstructive surgeries, as well as rotations in pediatrics, psychiatry, women’s health, and emergency medicine. As a Physician Assistant, she has had experience working in physical medicine orthopedics, hematology/oncology, and family medicine.
In her spare time, Tiffany enjoys spending time with her family, volunteering at her church, traveling, and doing outdoor activities with her dogs. She most importantly loves to read about new ways to help her patients in any way she can!

Courtney Fischer-Brown MPAS, PA-C is a native Houstonian. She graduated Magna Cum Laude with a degree in Physiology from Texas State University. She received her Master of Physician Assistant Studies from UT Southwestern Medical Center in Dallas, where she was inducted into the Pi Alpha National Physician Assistant Honor Society.
Courtney has been practicing medicine since 2012 and has worked in specialties of urology, hepatology and allergy/asthma/immunology prior to joining Houston Family Practice. Her areas of clinical interest include preventative health, patient education, and chronic disease management .
Courtney is certified with the National Commission on Certification of Physician Assistants and licensed by the Texas Medical Board. She is a member of the Texas Gulf Coast Physician Assistant Association and the American Academy of Physician Assistants.
In her spare time she enjoys running and spending time with friends and family.

My name is Jae Graham and I have had the privilege to work at Houston Family Practice since 2006. I am a certified Physician Assistant, graduated from Baylor College of Medicine’s Masters of science program in 1995. I have worked in Internal medicine and Family Practice since graduation. Prior to this, I was a registered dietician working at Methodist hospital in cardiology and oncology. I continue to have a strong interest in effects of diet on prevention and treatment of diseases. From my years of practice with dedicated physicians, I feel very well rounded in caring for all the myriad of issues that we see in Family Practice. I enjoy the many relationships I have developed with our patients over the years. I feel our clinic does an amazing job in creating a professional and caring atmosphere for our patients. I work closely with specialists that I have known and trusted for years; to help facilitate an efficient health care system for our patients. I am a native Texan. My home life is wonderfully busy with three children and my husband of 29 years. We all enjoy being active and outdoors as much as we can. My favorite activities are yoga, biking and walking. We enjoy having healthy home cooked meals together and trying new recipes often.
